Claimont Health are recruiting for a Community Care Coordinator / Community Psychiatric Nurse (Band 6) to join our multidisciplinary team working within the London Borough of Enfield, to provide effective and timely care for working age adults who present with functional mental health problems who are residing in the borough.

This role will be key in providing on-going care and support to people with mental health problems, their carer’s, and relatives and will liaise with colleagues within primary care, community health and social care services, private and voluntary organisations, and the wider mental health of adult services as appropriate.

Main duties of the job:

  • To assess care needs, devise care programmes, packages of care including risk assessment and implement and evaluate these in collaboration with individual service users and their carer’s.
  • To undertake duties designed to extend clinical and managerial expertise as delegated by their line manager when required.
  • Have a defined caseload and work closely with other members of the team including psychology, medical professionals, and pharmacology to ensure that the best quality of care is provided.
  • The work will be underpinned by a bio psychosocial model of care and this role will ensure that best practice psychosocial interventions are offered whenever possible.

What do we need from you?

  • You can offer full availability of 37.5 hours per week.
  • You will be NMC registered with at least 18 months or more experience in your chosen field.
